General Summary: Responsible for ensuring nursing practices and delivery of care is carried out per state and federal guidelines. To provide nursing leadership and oversight to the nursing units. *Position Responsibilities*: · Follow through on labs ensuring MD are aware and appropriate physician orders are received and entered in Matrix, and family members are notified in a timely manner. · Maintain medication rooms and stock rooms · Audit Medicare skilled notes, weekly skin checks and CNA documentation. · Responsible for weekly wound rounds and completion of weekly wound documentation. · Conducts unit/infection control rounds and audits ABT treatment regimen documentation and notify DON of discrepancies. · Review all incidents and ensures proper documentation is in place including, progress notes, physician orders and proper notifications have been made. Notifies DON/Risk Manager if there is discrepancy in the incident reports. · Reviews all new admission and re-admission charts for completion and accuracy. Identifies discrepancies and make the necessary updates and corrections. · Conducts daily audit of physician orders to ensure accuracy of MD and Nursing orders. · Checks the accuracy of documentation and Point of Care documentation. Provides training if needed ensuring consistency on documentation. · Coordinates care delivery with Pavilion Leadership, ALFs, Physicians, Vendors, Hospice, Residents and POA/HCS/Family members. · Reviews and provides nursing team with clinical recommendation utilizing critical thinking skills. · Conducts focus audits as instructed by the Director of Nursing. · Conveys information and guidance from the Director of Nursing to the nursing staff and vice versa. Ensure shift change huddles and walking round are completed. · Attends leadership stand-up meetings.
