Position Summary The RN Hospital Admissions is responsible for facilitating timely and effective hospice admissions, ensuring patients and families receive compassionate, informative, and seamless care. This role requires collaboration with healthcare professionals, internal teams, and community partners to identify and overcome barriers to admission, while maintaining compliance with regulatory standards and best practices in hospice care. The RN ensures accurate assessments, develops initial care plans, educates stakeholders on hospice philosophy, and contributes to patient transitions and care coordination. Essential Functions • Promote hospice services and philosophy to patients, families, healthcare professionals, and the broader community. • Evaluate hospice eligibility using Medicare Local Coverage Determination and clinical judgment. • Perform and document comprehensive patient assessments, including physical, psychosocial, spiritual, and functional needs. • Facilitate admission process, including physician order acquisition and communication. • Provide emotional support and education to patients, families, and caregivers. • Collaborate with physicians, referral sources, and interdisciplinary team members to ensure seamless patient care and transition. • Facilitates transfers from the Hospital to next level of care based on Medicare Conditions of Participation. Acts as the primary nurse designee while patient is physically in the hospital and communicates all changes as a member of the interdisciplinary care team including updating the patient’s plan of care. • Serve as primary nurse designee for hospital-based patients, updating care plans and coordinating changes. • Maintain confidentiality in accordance with HIPAA regulations. • Educate medical community and partners on hospice referrals and benefits of early referral. • Mentor and train new staff; provide performance feedback to leadership. • Maintain professional representation of hospice organization in all external and internal engagements. • Participate in hospital committees, public relations activities, and educational programs. • Uphold organizational policies and procedures, fiscal responsibility, and ethical nursing standards. • Perform other duties as assigned.
