Our *Patient Services* team works with an extraordinary network of over 500 specialty volunteer providers, the safety net primary care system, and local hospitals to connect patients to the medical care they need, while providing the wrap-around support needed to help patients survive and thrive. *JOB SUMMARY*: The *PRN RN Case Manager* supports the organization’s Specialty Care Patient Program, providing case management for special patient panels (Hematology and Complex Care), both through direct case management and in coordination of case roles across the team. With the Director of Nursing, this role ensures smooth operations in support of all uninsured neighbors referred for advanced diagnostics and specialty care from the community’s safety net primary care clinics and other partners. *RESPONSIBILITIES: *Reporting to the Director of Nursing, the PRN RN Case Manager directly provides medical case management and support for: Patients with complex health conditions, including: * Those with two or more systems impacted + two or more barriers to care identified across the Social Drivers of Health (SDoH); and * Patients referred with a suspected malignancy pending advanced diagnostic care to confirm or rule out malignancy. Patients with a diagnosed condition that is seen in Hematology, specifically those in the WeCareJax Hematology Clinic hosted twice per month. Nursing Clinical Advice for the Community Health Worker assigned to support the Health & Wellness Uninsured Patient Hospital Discharge Program, which often includes patients moving into a special panel. Support for special panel patient scheduling, follow-up, and case support, working with the Patient Services team. Transition of care management for complex cases, including suspected cancer that is ultimately unconfirmed and confirmed cancer cases that move through treatment to surveillance and survivorship. As available, be on site for the twice-monthly Healthy Pantry / Food as Medicine program, ensuring that their assigned patients are able to participate. Notifies the resident’s attending physician and next-of-kin when there is a change in the resident’s condition. Administers professional services such as; catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc., as required. Monitors seriously ill residents as necessary. Ensures that personnel providing direct care to residents are providing such care in accordance with the resident’s care plan and wishes. Assists the Director of Nursing and/or Infection Control Coordinator in identifying, evaluating, and classifying routine and job-related functions to ensure that tasks in which there is potential exposure to blood/body fluids are properly identified and recorded. EDUCATION & EXPERIENCE: Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program. On-the-job training provided.
