Operating under a collaborative practice agreement, this role focuses on reducing polypharmacy, enhancing medication adherence, and managing chronic diseases to improve patient outcomes and elevate quality performance metrics such as CMS Star Ratings and HEDIS scores. The manager partners closely with patients, physicians, advanced practice providers, nurses, and care teams to deliver evidence-based, individualized care that promotes safety, efficacy, and cost-effectiveness. This role includes approximately 3.5 days per week dedicated to direct patient care and 1.5 days allocated to administrative responsibilities, including oversight and management of pharmacists within the assigned multistate region. The schedule is expected to follow a cadence of 3 weeks focused on direct patient care, followed by 1 week of travel supporting ambulatory care pharmacists across the region. ESSENTIAL JOB DUTIES/RESPONSIBILITIES: Direct supervision of Ambulatory Pharmacists and support staff, including hiring, performance reviews, and development plans within their region. Program Oversight — Manages the ambulatory pharmacy program within their region and their own location/center. Quality & Metrics Leadership — Owns CMS Star Ratings and HEDIS performance at a regional level, including A1C and HBP, not just contributes to them, as well as enhancing medication related STARs impacts. Emergency Department Admit Nurse RN 7p-7a

Overview: The ED Boarder Nurse provides ongoing nursing care to patients who have been admitted to the hospital but remain in the Emergency Department while awaiting inpatient bed placement. Care is delivered in accordance with inpatient standards and includes comprehensive patient assessments, medication administration, IV therapy, wound care, and patient education. The nurse performs initial and ongoing reassessments and closely monitors patients for any signs of clinical deterioration to ensure timely intervention. The role also involves coordinating care with Emergency Department providers, hospitalists, specialists, case management, and bed placement teams to support continuity of care and facilitate timely patient throughput. The Registered Nurse (RN) provides nursing care to patients utilizing the nursing process and standards of practice for a professional nurse. The RN is expected to perform all staff nurse activities and function as charge nurse when assigned. The RN maintains overall accountability/responsibility for assigned patients, even though some activities/tasks may be delegated to Technical Nurses, Patient Care Associates, and/or Sitters. Assignments are performed appropriately to age and developmental level of patients being served. ACLS from the American Heart Association is required within six (6) months of hire or transfer for the following departments: 3 ICU-North (620480), 5N CCU (730301), Cardiac Rehab (733100), Non Invasive Cardiology North (733080), and Noninvasive Cardiac Lab (733000).ACLS from the American Heart Association is required within six (6) months of hire or transfer for Progressive Registered Nurses in Intensive Care Units.PALS from the American Heart Association is required within six (6) months of hire or transfer for the following departments: PICU (660401) and Trauma Center (713100).
