Submit a tally of patient care visits made each day. Participate in case conferences, discuss with the supervisor problems concerning the patients, and how they may best be handled. Discuss, with the appropriate supervisor, the need for the involvement of other members of the health team, such as the Home Health Aide, the Physical Therapist, the Speech Therapist, the Occupational Therapist, The Medical Social Worker, etc. Cooperate with other agencies providing nursing, or related, services to provide continuity of care and to implement a comprehensive care plan. Participate in staff development meeting. Continually strive to improve his/her nursing care skills by attending in-service education, through formal education, attendance at workshops, conferences, active participation in professional and related organizations, and individual research and reading. Participate in the development, and periodic revision, of the physician's Plan of Treatment and processes change orders, as needed. Submit clinical notes, within 48 hours, and progress notes and other clinical record forms outlining the services rendered. Participate in the patient’s discharge planning process. Maintain an on-going knowledge of current drug therapy. Adhere to federal, state, and accreditation requirements, including Medicare and Medicaid regulations. May be requested, by Clinical Manager, to fill in for the other nurses. COORDINATES THE ADMISSION OF A PATIENT TO THE AGENCY Conduct an initial, and ongoing, comprehensive assessment of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments, at appropriate time points. Obtain a medical history from the patient, and/or a family member, particularly, as it relates to the present condition. Conduct a physical examination of the patient, including vital signs, physical assessment, mental status, appetite, and type of diet, etc. Evaluate the patient, family member(s), and home situation, to determine what health teaching will be required. Evaluate the patient's environment to determine what assistance will be available, from family members, in caring for the patient. Evaluate the patient's condition, and home situation, to determine if the services of a Home Health Aide will be required and the frequency of this service. Explain nursing, and other Agency, services to patients and families, as a part of planning for care. Develop, and implement, the nursing care plan. May be requested, by the Clinical Manager, to fill in for other nurses who are on vacation or sick. PROVIDES SKILLED NURSING CARE AS OUTLINED IN THE NURSING CARE PLAN Nursing services, treatments, and preventative procedures, requiring substantial specialized skill and ordered, by the physician. The initiation of preventative and rehabilitative nursing procedures, as appropriate, for the patient's care and safety. Observing signs, and symptoms, and reporting to the physician: reactions to treatments, including drugs, as well as changes in the patient's physical, or emotional, condition. Teaching, supervising, and counseling the patient, and caregivers, regarding the nursing care needs and other related problems of the patient, at home. ASSUMES RESPONSIBILITY FOR THE CARE GIVEN BY THE HOME HEALTH AIDE Supervise, and evaluate, the care given by the Home Health Aide, as needed, and at a minimum of, once every 14 days. Submit, to the appropriate department/individual, written evaluations of the Home Health Aides who are providing service to the patients, in his/her geographical area. Participate in periodic conferences, with the Home Health Aide supervisor, concerning the Aide's performance. Chart those services rendered to the patient, by the staff nurse, and changes that have been noted, in the patient's condition and/or family and home situation, makes revisions in the nursing care plan, as needed, records supervisory visits conducted with the Home Health Aide, evaluates patient care and progress, and closes charts of discharged patients. Evaluate the effectiveness of her nursing service to the individual and family. Discuss, with the supervisor, the need for involvement of other members of the health team, such as the Home Health Aide, physical therapist, speech therapist, occupational therapist, social worker, etc. Obtain orders for home health aide service and submit referrals to appropriate personnel. Provide guidance, and supervision, to the LPN and supervises the LPN, once monthly. Cooperate with other agencies providing nursing, or related, services to provide continuity of care and to implement a comprehensive care plan. Participate in the educational experiences for student nurses. Participate in the planning, operation, and evaluation of the nursing service. Prepare the care plan for the Home Health Aide. JOB CONDITIONS Must have a driver’s license and be willing, and able, to drive to patients’ residences. The ability to access patients’ homes, which may not be routinely wheelchair accessible, is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care. Physical activities will include, walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients. Part-Time HOME HEALTH & ADULT DAY HEALTHCARE - NURSE JOB DESCRIPTION Reports to: Home Health Manager / ADHC Manager Department: Home Health / ADHC Classification: Exempt Division: Home Health / ADHC Date: June 19, 2026 JOB SUMMARY: This position provides part-time (ranging from 8-24 hours per week) RN support to NCCOA’s home health and adult day health care departments. In conjunction with the Home Health Manager, the Home Health & Adult Day HealthCare (ADHC) Nurse (The RN) provides nursing supervision for home health clients and staff, conducts timely, in-home client intake and/or recertification assessments, and provides training oversight to Certified Nursing Assistants and Home Health Aides. In conjunction with the ADHC Manager, responsibilities also include overseeing and facilitating a welcoming, nurturing, stimulating and enjoyable daily program of therapeutic activities as well as staff assistance with activities of daily living (ADLs) for seniors with chronic illness, dementia, Alzheimer’s disease and other cognitive deficits. DUTIES AND RESPONSIBILITIES: The RN conducts home visit/assessments as assigned by the Home Health Manager, and supports the ADHC Manager to oversee the ADHC facility’s day-to-day operations: Responsible for and in charge of the ADHC facility and its staffing and present on the ADHC premises during the ADHC Club hours of operation. If the RN is not present, the RN ensures that a qualified person and/or Club Manager is present during the ADHC hours of operation. The RN is responsible and accountable for the administration of all components of the ADHC facility, and the implementation and enforcement of all NCCOA policies and procedures, standards of care and program development in accordance with the needs and social, physical and mental capabilities of the members served. Knowledgeable and keeps updated with Agency for Health Care Administration (AHCA), Department of Elder Affairs (DOEA) and any other applicable governing bodies’ adult day care center regulations. Ensures that ADHC staff comply with the current NCCOA Employee Handbook and NCCOA policies and procedures at all times including but not limited to areas of cleanliness and hygiene, behavior and conduct, attendance and illness, training and housekeeping as well as comply with all applicable AHCA, DOEA and any other applicable governing bodies’ regulations. Maintains a daily schedule of staff assignments, ensuring that a minimum staff ratio of 1:5 is maintained at all times, and no less than two (2) staff members, at least one of whom is certified in an approved First Aid and CPR course, are present at all times during ADHC hours of operation. Ensures that all activities, documentation and records comply with all regulatory standards including but not limited to Major/Adverse Incident Reports, Member Travel Time Records, Member Enrollment Logs, Fire Drill Reports, Cleaning Logs, Bathroom Logs, Nutrition/Meal/Snacks and Fluids Logs, Vital Signs Logs, Staff Assignment records, Volunteer Logs, Member File Notes, Travel Time records, attendance records, average daily attendance, Refrigerator/Freezer Temperature Logs, number of participants enrolled, inspection reports, Planned Program Activities Calendars, job descriptions, advertisements, licenses and training records. Maintains all electronic logs/templates and data on the Company Share drive, accessible to the ADHC staff and Training & Caregiver Services Director at all times as well as maintains member records in paper and/or secure electronic form. Responsible for all day-to-day food service to ADHC members, coordinating and supervising food services with other ADHC activities and services, ensuring that ADHC staff perform their duties in a safe and sanitary manner, being knowledgeable of foods that meet regular diet requirements, participating in ongoing orientation and training and participating in biennial in-service provided by a registered dietician. Ensures morning snack, lunch and afternoon snack is offered to members in compliance with regulations. Obtains monthly lunch menu from the Nutrition Manager. Maintains and ensures a snack calendar, as well as a copy of the lunch menu, are posted in the ADHC facility and available to staff and caregivers upon request. Interviews and assesses prospective members with their caregivers for suitability for the ADHC Program, completing documentation as required, making every effort to enroll suitable members who can benefit from ADHC services, to fully utilize the center’s capacity without exceeding approved maximum facility capacity. Reviews and updates member’s care plan at least quarterly. Oversees, supervises and assists with the preparation and implementation of monthly activities calendar for members/caregivers and provides a copy to the Marketing department to post on the NCCOA website. The activities calendar is posted in the ADHC facility and is provided to caregivers. Ensures that daily activities as listed on the activities calendar, covering 70% of the center’s hours of operation, are therapeutic, social and health-enhancing to help restore, remediate or maintain optimal functioning and increase the interaction of members. These activities include but are not limited to exercise, health screening, health education, interpersonal communication and behavior modification, leisure activities or spectator programs for self-expression, self-esteem and mental stimulation, social participation, including arts and crafts, hobbies, excursions, outings, picnics or cookouts, self-care training activities for ADLs, rest, food, nutrition education and counseling. Recruits volunteers to assist with activities and field trips/outings, working with the Volunteer Manager to ensure agency policies and procedures are followed for ADHC volunteers. Ensures that ADHC staff and caregivers are familiar with the Emergency Management Plan (EMP) and procedures and are given a copy. Assists caregivers to register members for online EMP registration with the Nassau County Department of Health when needed. Conducts regular Fire Drills in compliance with all regulations, policies and procedures, and maintains records of all drills. Ensures that all staff is familiar with and the caregivers have a copy of the Silver Alert policy and procedures. Performs individual and group counseling to members’ caregivers as well as the community when needed. Markets the ADHC program to the community by identifying and reaching out to prospective members, caregivers and providers in the continuum of care, distributing business cards, brochures and other materials. Responsible for creating and maintaining all reporting log templates required for documentation, ensuring that all member files are complete with all information and forms and in compliance with all regulations. Ensures that all necessary supplies are available for the ADHC center including craft and activity supplies, consumables, food service and cleaning supplies. Organizes and assigns staff to daily, weekly and monthly ADHC cleaning tasks and ensures completion of all tasks. KNOWLEDGE, SKILLS AND ABILITIES: RN must be knowledgeable about the array of community-based services and resources available to address the needs of members and their caregivers with chronic illnesses including Alzheimer’s disease and other dementias. CPR and First Aid Certified. Must have knowledge of obstacles facing the aging and vulnerable population. Excellent verbal communication and organizational skills. Excellent writing skills. Microsoft Office skills (Word, Excel, PowerPoint, etc.). Self-directed and self-motivated. Professionalism SUPERVISION: Performs duties under the supervision of the Home Health Manager &/or Adult Day Healthcare Manager. WORK EXPERIENCE/REQUIREMENTS: At least one (1) year of staff supervisory experience in a social or health care services environment, and a minimum of one (1) year of experience in providing services to persons who have Alzheimer’s disease and other dementias. Experience working with the elderly or other vulnerable populations in a customer-focused environment. Registered Nurse (RN) or Licensed Practical Nurse (LPN) active license and able to work as a nurse in the state of Florida. Must pass the Level 2 background check required by DOEA and AHCA. Negative TB test. HIPAA trained and certified. Alzheimer’s disease Initiative trained and certified (4 hours). Certified Food Safety Manager recommended. Must adhere to Drug-Free, Smoke-Free and Alcohol-Free Workplace standards. Must have use of a personal vehicle for marketing, outreach and home visiting purposes when needed. Must have a valid driver’s license. When working in the Adult Day Health Center, RN must be physically present in the ADHC facility at all times unless the Manager arranges for adequate staff coverage. Lunch is taken in the facility with the members. Required facility staffing must be maintained at all times. PHYSICAL WORK REQUIREMENTS: Able to sit and work at a computer keyboard for extended periods of time. Able to stoop, kneel, bend at the waist and reach on a daily basis.
