Position Summary The RN Case Manager is responsible for planning and managing the nursing care of an assigned caseload of patients in home or long-term care settings. In collaboration with the interdisciplinary team, the RN Case Manager ensures that patient- and family-directed goals are achieved using a holistic, patient-centered approach. The role includes providing education, facilitating communication, and coordinating care across various sites. Flexibility in work location across care settings is required. Essential Functions • Assess and identify patient and family goals and needs. • Collaborate with interdisciplinary team members to develop and implement individualized care plans. • Participate in team meetings and ethical discussions. • Educate patients and caregivers about disease progression, medications, nutrition, and hospice philosophy. • Coordinate timely and effective responses to changing or urgent patient needs. • Maintain accurate, timely, and compliant documentation. • Support and educate team members, patients, families, and the broader community. • Participate in internal quality improvement initiatives. • Facilitate interdisciplinary group (IDG) meetings in the absence of the manager. • Perform other duties as assigned. Qualifications Knowledge and Critical Skills • Effective verbal, written, and non-verbal communication skills. • Understanding of hospice and palliative nursing principles. • Cultural competence and ethical decision-making in care delivery. • Ability to set professional boundaries and resolve team conflicts. • Strong critical thinking and clinical judgment abilities. • Proficient in time management, organization, and accountability. • Knowledge of infection control and standard precautions. Experience • Minimum of one year of Registered Nurse experience. • Preferred: One year of experience in a hospital medical-surgical or oncology setting. • Acceptable alternative: Five years of Licensed Practical Nurse experience within the past five years, including one year of hospital medical-surgical or oncology experience License / Certification • Current Registered Nurse license in the State of Florida. • Valid CPR certification. • Current driver’s license with an acceptable driving record. Education • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited nursing program. Other Requirements • Medical clearance for communicable diseases. • Up-to-date immunizations (e.g., TB, Hepatitis, Flu). • Ability to work extended and irregular hours as needed. • Ongoing participation in professional development and continuing education. GENERAL SUMMARY: The Resident Care Coordinator provides direct supervision of care giving within the community. Ensures residents are treated with respect and dignity recognizing individual needs and encouraging independence. Fosters a homelike atmosphere throughout the community. PRINCIPLE DUTIES: Essential Job Duties: 1. Provides direct supervision of resident care staff, which includes monitoring job performance, conducting associate performance evaluations, coaching and counseling employees and conducting and documenting corrective action needed. 2. Coordinates training and education of resident care staff on an ongoing basis. Assists in facility preservice orientation. 3. Verifies all employee hours on time cards and weekly hours report. Completes nursing schedules to assure proper coverage to meet resident needs and according to budget. 4. Assigns monthly weights to rotating shifts. 5. Monitors staff in administration of medication, to include frequent audits of medication sheets. 6. Assists the Director of Nursing in medication management, including the assistance of self-administration of medications to residents, ordering medication from pharmacy and maintaining records as dictated in community policies and procedures manual. Ensures that medications are properly stored, medication carts/cupboards are cleaned, stocked, and medication room is organized according to standards. 7. Observes residents’ health status, takes actions to address concerns within scope of practice, and reports all significant changes, reactions to medications and treatment or significant incidences to Assisted Living Director and Director of Nursing in a timely manner. Reviews capabilities of resident who self-administer medications on regular basis (refer to state regulations regarding medication assistance and documentation). 8. Conducts pre-admission screening of prospective new residents when assigned by supervisor and updates every six months or as needed. Assures that all required documentation, including service plans and nursing assessments, is completed. QUALIFICATIONS: 1. Associates degree in health care related field and/or 1-2 years’ related experience 2. State-qualified training in medication assistance, if applicable.
