Licensed Practical Nurse – University Inpatient Rehab Hospital-FT Nights

A Licensed Practical nurse performs nursing functions under the direction of a registered nurse, follows the plans of care, participates in care planning, and performs nursing actions in accord with company policy and state law. Key Responsibilities: Documents, observations and care are given in accordance with company process and company policies and procedures. Administers medications. Administers basic nursing care measures to the rehab patient and assists in giving care to the acutely ill rehab patient relative to patient age, developmental needs, and understanding. Admits, transfers, and discharges patients. Observes, records, and reports subjective patient symptoms. Assists the RN in giving physical and emotional support to the patient. Provides for dietary and fluid needs of the patient such as nourishment, feeding, tube feeding, observing and recording IV fluids and maintaining an accurate fluid intake and output. Takes and record vital signs: temperature, pulse, respirations, and blood pressure. Each schedule will cover a 4-week period of time. All Division Nurse Float Team members are obligated to be scheduled 4 weekend shifts per schedule period (every other weekend, or every Saturday, or every Sunday). Each staff member is obligated to work a minimum 2 Fridays, 2 Mondays, or a Friday and a Monday per schedule period. The Quality Outcomes Specialist (QOS) is responsible for improving the understanding, knowledge base, expertise and quality of OASIS scoring and home health documentation through the mentoring of home health staff. Job Responsibilities: Responsible for maintaining active participation with assigned teams by attending case conferences, huddle calls and other staff meetings. Responsible for monitoring and improving the outcome performance of assigned Regions and specific clinicians. Implements the Quality Outcomes Mentor Program for appropriate candidates using SHP scorecards, results from chart audits and staff identified by CTMs, Clinical Educators and Preceptors. Provides professional support to mentorship program at an assigned office location or on a patient visit and may have to travel within the assigned region to provide mentorship and/or training. Works with clinicians to master OASIS accuracy and documentation that supports medical necessity and homebound status by reviewing patient documentation including OASIS, referral, history, and physical and clinical notes. Consults with the clinician to clarify any data integrity issues and/or corrections to documentation; documents the same in the medical record and makes necessary corrections to the assessment. This may involve contacting the field RN/PT/OT/ST via telephone or electronic media to gain understanding of diagnostic assessment details. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities and in developing the plan of care. Works with clinicians to obtain and master knowledge of how to complete a plan of care that is pertinent to the patient’s clinical social and emotional needs while promoting improved patient outcomes. May assist with other chart audit activities as needed. May monitor open and closed charts and may participate in regular clinical record reviews. Overview: The Perioperative Registered Nurse (RN) provides nursing care to patients in select phases of Perioperative care (as competency demonstrates). This care utilizes the nursing process and the standards of care of a professional nurse. The RN is expected to perform all staff nurse activities and functions as charge nurse when assigned. The RN maintains overall accountability/responsibility for assigned patients, even though some activities/ tasks may be delegated to Surgical Technologists, Patient Care Associated, Guest Associates and Anesthesia Technologists. Under the supervision of the Nurse Manager, the RN manages individualized, goal directed nursing care through use of the nursing process and the principles of primary nursing in accordance with departmental and hospital policies and procedures Qualifications: Age of Patients Served: All Age Groups. Required Education: Associate's Degree Specific course work: Graduate of an accredited school of nursing. New hires effective June 14, 2017, must have a BSN degree from an accredited school of nursing within three (3) years of hire or transfer into the position.
