*Job Overview* Bravery is a new, employee centric skilled home care company in St Augustine, FL. As we open our doors to clinical care, we are looking for those not scared of the journey, passionate about the outcome, and inspired to be more in the roles they fill. All positions will be considered, fulltime, PRN, and part-time. Serious inquiries only and those that are in pursuit of a _Brave _new chapter. *Duties* * Conduct thorough assessments of patients' health conditions and needs. * Develop and implement individualized care plans based on patient assessments. * Administer medications and treatments as prescribed, including injections and tube feeding. * Monitor vital signs and document changes in patients' conditions. * Provide specialized care for patients with specific needs such as dementia care, or ostomy care. * Educate patients and their families about health management and disease prevention. * Collaborate with physicians and other healthcare professionals to coordinate patient care. * Maintain accurate medical records in compliance with ICD-10 coding standards and Medicare guidelines. *Skills* The successful candidate will demonstrate the following skills: * Proficient in dementia care, wound care, and chronic disease management techniques and strategies. * Proficient in post-operative and post-acute inpatient care. * Experience in administering injections safely and effectively. * Knowledge of tube feeding procedures. * Ability to assess and monitor vital signs accurately. * Familiarity with ostomy care practices.
