Well&You RuralReach will open four permanent satellite clinics across Northeast Florida, including rural Baker and Nassau counties, bringing chronic disease management, vascular and limb-salvage care, cardiac monitoring, women's health, and non-opioid pain care to residents who now drive 35 to 40 miles for specialty care. In communities where heart disease, diabetes, stroke, and amputation rates run well above Florida's average, this award turns Well&You's long-standing local roots and relationships into permanent access points for care. Staffed by board-certified physicians and APRNs, these clinics will build sustainable rural health infrastructure and bring a better way to care to rural Northeast Florida.
*Job Summary:*
Registered Nurses (RN) in our rural health program deliver direct, hands-on clinical care in the outpatient clinic setting. The role covers wound care and compression management, diagnostic testing including 12-lead EKGs and ambulatory cardiac monitoring, medication and injection administration, point-of-care testing, telephone and in-person triage, chronic disease education, and post-procedure follow-up for vein, vascular, cardiovascular, and pain management patients. At rural sites the RN is frequently the senior clinical presence on the floor, working alongside a Medical Assistant and technologist with the physician occupied or available remotely. The role therefore requires an experienced nurse who exercises independent clinical judgment at the top of licensure, recognizes when a finding cannot wait, anticipates what a remote site will need before the clinic day starts, and serves as a clinical resource and teacher for the staff working alongside them.
*Duties/Responsibilities:*
* Perform comprehensive wound assessment and treatment, including cleansing, dressing selection and changes, debridement within scope and protocol, and management of venous, arterial, diabetic, and post-procedure wounds.
* Apply and manage compression therapy, including multilayer wraps, compression stockings, and lymphedema management, and educate patients on adherence between visits.
* Measure and document wound progression over time, escalate non-healing or deteriorating wounds promptly, and coordinate advanced therapies or referral when healing stalls.
* Perform 12-lead EKGs, verify tracing quality, recognize abnormal and life-threatening findings, and escalate immediately per protocol.
* Place, remove, and troubleshoot remote blood pressure monitors and event monitors; instruct patients on wear, symptom logging, and return of equipment.
* Perform vital signs, oxygen saturation, and other clinical measurements; recognize and act on values outside expected parameters.
* Administer medications and injections, including intramuscular, subcutaneous, and intradermal, and establish intravenous access and administer infusions per provider order and protocol.
* Perform specimen collection, labeling, and handling; run point-of-care testing and document results.
* Conduct telephone and in-person triage, using independent nursing judgment to determine what can be managed at the clinic, what needs same-day provider attention, and what requires emergency care.
* Provide post-procedure follow-up and wound checks in clinic and by phone for patients who cannot easily return.
* Deliver patient and family education on wound care, medications, compression, chronic disease management, and follow-up, adapted to health literacy and home circumstances.
* Prepare rural patients for upcoming care by confirming records, imaging, labs, medications, and clearance requirements in advance so a patient does not travel hours for a visit that cannot proceed.
* Verify before every rural clinic day that emergency equipment, medications, and supplies are present, in date, and functional, and resolve gaps in advance rather than at the bedside.
* Recognize and lead the response to clinical emergencies at sites where advanced backup and transport are farther away, including activation of emergency services and stabilization until transport arrives.
* Coordinate clinical handoffs and escalation with rural referral partners, including critical access hospitals, rural health clinics and FQHCs, primary care practices, home health, wound care centers, and EMS.
* Serve as clinical resource and preceptor for Medical Assistants and technologists at rural sites; train, validate competencies, and reinforce protocol adherence.
* Ensure the clinical environment, instruments, and supplies are clean, sterile where required, and safe, following infection control protocols.
* Maintain accurate and timely documentation in the electronic health record, including assessments, interventions, wound measurements, diagnostic results, and outcomes.
* Follow policies, procedures, and regulatory guidelines to ensure compliance with safety and quality standards.
* Contribute to rural grant quality and access measures, including care-gap closure, screening events, and complete capture of clinical and outcome data.
* Travel to assigned rural clinic sites and community outreach and screening events.
*Required Skills/Abilities: *
* Strong critical thinking and independent problem-solving; confident practicing at the top of licensure with limited immediate backup.
* Deep clinical knowledge foundation in wound care and compression management, including wound etiology, dressing selection, and recognition of infection and deterioration.
* Competence performing and evaluating 12-lead EKGs, including recognition of ischemia, arrhythmia, and other findings requiring immediate escalation.
* Proficiency in medication and injection administration, IV access, venipuncture, and point-of-care testing.
* Solid understanding of venous, arterial, cardiovascular, and chronic disease processes sufficient to educate patients and anticipate complications.
* Sound independent judgment on triage, escalation, and transfer decisions.
* Proven ability to work safely and resourcefully in resource-constrained settings, improvising within policy, scope, and standard of care.
* Ability to independently prioritize competing demands across a full clinic day with a small team.
* Comfort teaching, precepting, and directing less experienced clinical staff.
* Excellent communication and interpersonal skills, and the ability to work effectively as part of a small team.
* Proficiency with telehealth workflows and EHR documentation over variable connectivity.
* Cultural competence and ease with rural patient populations, including patients with limited health literacy or long-standing gaps in care.
* Willingness and ability to travel to rural clinic sites and outreach events on a regular basis.
*Education and Experience:*
* Active Registered Nurse license in the state of Florida
* Minimum 3–5 years of RN experience in an outpatient clinic, wound care, cardiology, vascular, or ambulatory setting
* Demonstrated hands-on wound care and compression therapy experience required
* Demonstrated competence performing 12-lead EKGs required
* Experience with Wound Care Dressing Changes
* Ability to place IVs
* Strong patient outreach, communication skills, and ability to critically think
* Electronic EMR experience
* Experience in a rural, critical access or outreach nursing setting strongly preferred
* Demonstrated history of working independently with minimal on-site supervision
* Current BLS certification required; ACLS preferred
* Valid Florida driver’s license, reliable transportation, and willingness to travel to rural sites and outreach events
*Physical Requirements: *
* Standing and walking for long periods, assisting with patient positioning and transfers, and handling clinical equipment.
* Must be able to lift up to 15 pounds at times.
* Frequent bending, kneeling, and reaching to perform wound care and lower-extremity treatment.
* Potential exposure to communicable diseases, bodily fluids, and wound drainage.
* Regular travel between the Jacksonville area and rural clinic sites, including some early start times and occasional evening or weekend outreach events.
* Must be able to load, transport, and set up portable equipment, supplies, and event materials for offsite clinics and community screening events.
* Must be able to work productively in smaller rural facilities and temporary event spaces with limited support staff on site.

This position is funded in whole or in part by a rural health grant. Continued employment in this role is contingent on grant funding and on the program performance and reporting requirements attached to that funding.
Pay: From $44.00 per hour
