More information: https://info.flclearinghouse.com MDS Coordinator Summary Description: The MDS Coordinator under the direction of the Administrator or Director of Nursing, is responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and development of the Interdisciplinary Plan of Care. Additionally, this individual will lead the daily Medicare Prospective Payment System (PPS)/Medicaid Case mix Meeting. These duties are to be performed in a timely manner consistent with Centers for Medicare Medicaid (CMS) guidelines and policies and procedures. This individual also establishes and maintains positive relationships with all members of the healthcare team. Essential Functions and Responsibilities: * Maintains current MDS status of each patient/resident, in accordance with all state and federal guidelines, including the Omnibus Budget Reconciliation Act (OBRA), Medicare PPS and any Medicaid payment system, as applicable. * Schedules patients/residents for appropriate OBRA, Medicare PPS and/or Medicaid required assessments. * Communicates schedule to all other departments and families, as required. * Monitors clinical systems for changes in patient/resident condition. * Responds to patient/resident change in condition by coordinating patient/resident re-assessment and re-evaluation plan of care. * Strategically sets assessment reference date (ARD), in conjunction with the interdisciplinary team, to capture needs, care and services delivered to most accurately reflect the status of the patient/resident. * Accurately completes portions of MDS and CAAs as assigned, in accordance. with policies, procedures, and guidelines. * Assures compliance with federal, state, Medicare and Medicaid regulations, and facility policies and procedures as related to completion of an accurate electronic submission in a timely manner and coordination of the RAI process. * Coordinates and develops plan of care as required. * Monitors MDS and care plan documentation on all patients/residents, assuring that complete, orderly and chronological documentation is maintained in the file. * Maintains Medicare Certifications /Re-Certifications * Participates in QAPI /QM process * Maintains proficiency in software programs for MDS completion and is able to access and utilize appropriate reports.

Essential Functions • Conducts comprehensive and ongoing nursing assessments of patients, identifying physical, psychosocial, emotional, and spiritual needs to inform individualized care planning. • Collaborates with the interdisciplinary team to develop, implement, and regularly update patient- and family-centered plans of care that align with the patient's goals, condition, and hospice philosophy. • Provides direct nursing care, including medication administration, symptom management, wound care, and other treatments in accordance with physician orders, hospice protocols, and established standards of practice. • Educates patients, families, and caregivers about disease progression, symptom control, medication usage, nutrition, hydration, and the hospice approach to end-of-life care. • Maintains accurate, timely, and complete documentation of patient assessments, interventions, communications, and outcomes in accordance with organizational policies and regulatory requirements. • Communicates effectively and compassionately with patients, families, caregivers, and members of the interdisciplinary team to ensure continuity and quality of care. • Provides emotional support and anticipatory guidance to patients and families throughout the dying process and offers appropriate closure following the death of a patient. • Oversees the care provided by hospice aides and other support staff, ensuring that delegated tasks are performed in alignment with the patient’s plan of care and quality standards. • Participates in care conferences, shift hand-offs, and team meetings to ensure shared understanding of patient needs and progress. • Implements and adheres to infection prevention and control practices, including standard precautions and proper handling of equipment and bodily fluids; educates others as needed. • Responds promptly and effectively to urgent or emergent changes in patient condition, initiating appropriate interventions and communicating with providers to obtain necessary orders. • Applies clinical judgment and critical thinking skills in planning and prioritizing care in a fast-paced inpatient environment. • Demonstrates proficiency in using clinical technology, including electronic medical records, medication dispensing systems, and monitoring equipment. • Maintains required certifications, demonstrates clinical competency, and actively participates in training and development opportunities. • Upholds professional standards and maintains therapeutic boundaries in all interactions with patients, families, and colleagues. • Performs other related duties as assigned to support the efficient and compassionate delivery of inpatient hospice care. Responsibilities The Surgical Technologist provides safe and efficient care to the surgical patient under the supervision of the registered nurse (RN) and in collaboration with the surgeon. Concierge Home Care is seeking an experienced Registered Nurse (RN) to provide skilled evening home health visits throughout Clay County. This role is ideal for nurses who enjoy working independently while helping patients remain safely at home. Coverage includes: Evening Start of Care admissions Skilled nursing visits Urgent nursing visits (Up to 10:00 PM) Resumption of Care visits Minimum Productivity: 24 points per week What You'll Do Provide skilled nursing care to patients in their homes while ensuring continuity of care after normal business hours. Responsibilities include: Complete Start of Care (SOC) admissions Skilled nursing revisits Resumption of Care (ROC) visits PRN and urgent nursing visits Perform comprehensive patient assessments Medication reconciliation Develop and update individualized care plans Wound care Disease management education Catheter care IV starts Foley catheter management Nephrostomy care Infection prevention education Monitor urinary retention Post-surgical urology care Coordinate care with physicians and interdisciplinary teams Complete OASIS documentation and EMR charting Participate in rotating evening/weekend on-call coverage What We're Looking For Required: Active Florida RN License or Compact RN License Valid driver's license Reliable transportation Current auto insurance Preferred Experience: Home Health OASIS Documentation Skilled Nursing Med-Surg Acute Care Hospice Foley catheter management Successful nurses are: Comfortable working independently Organized and self-motivated Excellent communicators Strong clinical decision-makers Compassionate patient advocates Why Nurses Love Concierge Home Care At Concierge Home Care, we believe clinicians deserve flexibility, support, and the tools to succeed.
