Nurse Practitioner – Sickle Cell Center & Infusion Services: Jacksonville

Foundation for Sickle Cell Disease Resea | Jacksonville, FL 32216

The Foundation for Sickle Cell Disease Research (FSCDR) is a growing, multi-site clinical organization committed to making sickle cell disease a manageable, livable condition throughout the State of Florida. We provide integrated, comprehensive and patient-centered care through a multidisciplinary team of physicians, nurse practitioners, registered nurses, medical assistants, case managers and operational professionals. We are seeking a nurse practitioner who wants to do more than simply inherit an existing patient schedule. This is an opportunity to help strengthen and grow a local clinical practice while providing high-quality care to patients with complex needs. Position Summary The Nurse Practitioner will provide direct patient care, support infusion and treatment services, maintain clinical readiness at the assigned location and help strengthen patient engagement and continuity of care. The NP will serve as the *local clinical anchor* of the practice while reporting to and receiving guidance from FSCDR’s Chief Nursing Officer, collaborating physicians and supervisory advanced practice providers. This is not a narrowly transactional position in which the provider sees only the patients already on the schedule and disengages when the final visit is completed. Patient volume may vary as the location grows. During lower-volume periods, the NP is expected to remain actively engaged by reviewing the local patient panel, following up on clinical needs, helping reconnect patients who are overdue for care, closing documentation and order-related tasks, supporting the Network clinical team, monitoring site readiness and contributing to practice-growth initiatives. The successful candidate will be self-directed, accountable, and collaborative. RN and medical assistant support will be available; however, every member of the site is expected to work together when patient care or clinic operations require assistance. Primary Responsibilities Direct Patient Care * Conduct comprehensive and focused patient evaluations, including medical histories, physical examinations, medication reconciliation and review of relevant clinical records. * In tandem with the Physician, manage acute and chronic conditions within the NP’s education, certification, licensure, privileges and organizational protocols. * Evaluate patients presenting with pain, dehydration, anemia-related symptoms, infection concerns, medication complications and other acute or chronic issues. * Recognize signs of clinical deterioration and initiate appropriate stabilization, physician consultation, emergency response or transfer to a higher level of care. * Order and review laboratory testing, diagnostic studies and other clinically appropriate services. * Make timely referrals and coordinate care with physicians, specialists, pharmacies, care managers and other members of the patient’s healthcare team. * Provide patient and family education concerning medications, treatment plans, disease management, warning signs, preventive care and appropriate use of available services. * Administer or order vaccines, hydration, supportive treatments and other medically appropriate services within scope of practice. * Prescribe medications, including controlled medications when clinically indicated and * Verify medication selection, dosage, route, frequency and patient-specific considerations before transmitting prescriptions or treatment orders. * Promptly correct rejected, incomplete or inaccurate prescriptions and orders to prevent unnecessary interruption in patient care. * Close the loop on abnormal results, medication issues, referrals, patient messages and other clinical follow-up needs. Infusion and Procedural Care * Assess patients before infusion or treatment and determine whether treatment may proceed safely under the applicable order, physician protocol and plan of care. * Provide or oversee hydration, medication infusions, supportive therapies and other approved outpatient treatments. * Demonstrate competency in peripheral venous access and implanted port access or successfully complete FSCDR’s competency validation before performing these functions independently. * Use appropriate aseptic technique and infection-prevention procedures when accessing vascular devices, preparing medications and administering injections or infusions. * Monitor patients during and after treatment and respond appropriately to changes in condition, adverse reactions, oversedation, respiratory compromise or other emergencies. * Coordinate effectively with the RN and medical assistant to ensure safe patient flow, appropriate monitoring and completion of treatment documentation. * Follow policies governing medication receipt, preparation, administration, reconciliation, storage, security, wastage and documentation. * Maintain readiness to initiate BLS or ACLS interventions and activate emergency medical services when required. * Participate in ongoing competency validation, emergency drills, medication-safety education and infusion-related training. Patient Engagement and Practice Growth * Use eClinicalWorks and other approved systems to understand and actively manage the assigned location’s patient list. * Identify established patients who are overdue for follow-up, have unresolved care needs, have missed appointments or may benefit from clinically appropriate services. * Participate personally in patient outreach when a clinical conversation, education, reassurance or treatment decision is needed. * Work with the RN, medical assistant, scheduling staff and care-management team to reconnect patients, schedule appropriate follow-up and reduce preventable gaps in care. * Educate patients about FSCDR’s available clinical, infusion, preventive and supportive services when those services may be appropriate to their needs. * Help improve appointment completion, continuity of care, schedule utilization and patient retention without compromising medical necessity or quality. * Support local referral development by communicating professionally with community providers and helping ensure referred patients receive responsive follow-up. * Represent FSCDR at occasional community education, screening, outreach or awareness events. * Help identify barriers preventing patients from receiving care and communicate those barriers to the appropriate clinical or operational leader. * Contribute ideas for expanding responsible access to care and improving the patient experience at the assigned location. Local Site Stewardship * Function as the primary on-site advanced practice provider and local clinical point person for the assigned location. * Help lead brief daily huddles and communicate the day’s clinical priorities, treatment needs, staffing considerations and potential safety concerns. * Collaborate with the RN and medical assistant to organize patient flow and use team members effectively within their respective scopes of practice. * Maintain awareness of medications, infusion supplies, clinical supplies, emergency equipment and other resources necessary for safe operations. * Ensure that inventory counts, expiration-date reviews, refrigerator logs, emergency-equipment checks and related clinical-readiness tasks are completed and documented. * Escalate medication, staffing, equipment, supply, facility or operational concerns before they create an interruption in patient care. * Maintain an orderly, professional and patient-ready clinical environment. * Communicate regularly with the Chief Nursing Officer regarding site activity, clinical concerns, patient needs, safety events and barriers to growth. * Participate in site, nursing, quality and operational meetings as required. * Follow through on assigned action items without repeated prompting. * Assist with implementation of new workflows, clinical services, technology and practice-improvement initiatives. * Participate in incident review, root-cause analysis and corrective-action activities when applicable. Professional Accountability and Teamwork * Demonstrate a consistently respectful, solutions-oriented and professional attitude toward patients, colleagues and leadership. * Accept responsibility for one’s own clinical decisions, prescriptions, orders, documentation and follow-up. * Respond promptly when a time-sensitive clinical concern, order correction or patient-safety issue associated with the NP’s work is identified. * Participate in FSCDR’s written escalation, coverage and after-hours response procedures. * Communicate early when assistance, clarification or escalation is needed rather than allowing an issue to remain unresolved. * Remain engaged during lower-volume periods and independently identify productive, patient-centered work that supports the site. * Step in within the NP’s role and scope when another team member needs reasonable assistance. * Give and receive constructive feedback professionally. * Support a team culture in which patient care is shared, problems are addressed and no team member is knowingly left unsupported during a clinically demanding situation. Documentation, Quality and Compliance * Complete accurate, thorough and timely documentation in eClinicalWorks, preferably on the date of service. * Maintain and close EHR inbox items, refill requests, results, telephone encounters and other assigned tasks within established timelines. * Ensure documentation supports the services performed and the clinical decisions made. * Follow applicable state scope-of-practice requirements, prescribing requirements, DEA requirements, HIPAA, OSHA, infection-control standards and FSCDR policies. * Participate in chart review, quality audits, competency assessments and peer-review processes. * Protect patient confidentiality and use only approved systems for clinical communication. * Report clinical incidents, medication errors, near misses and safety concerns promptly and transparently. * Participate in continuing education related to sickle cell disease, infusion therapy, medication safety and emergency response. Required Qualifications * Master’s or doctoral degree from an accredited Nurse Practitioner program. * Current, unrestricted Nurse Practitioner/APRN license in State of Florida. * Current national board certification appropriate to the population served, such as FNP, AGPCNP, AGACNP or PNP. * Current unrestricted Registered Nurse license where required by the applicable state. * At least *four years of experience practicing as a Nurse Practitioner* in direct patient care.
