Overview: As a member of the treatment team, the Licensed Practical Nurse will provide direct patient care to assigned patients under the direction and supervision of a Registered Nurse and attending clinician within the Licensed Practical Nurse (LPN) scope of practice. Key Responsibilities Patient Care & Clinical Duties Administer medications and treatments as prescribed Perform clinical procedures such as catheterization, tube feedings, suctioning, and wound care Monitor and record vital signs and patient conditions Team Supervision Oversee and direct daily activities of Nursing Assistants Ensure compliance with facility policies and high standards of care Documentation & Compliance Maintain accurate and timely patient records Document responses to treatment and care plans Process physician orders, admissions, transfers, and discharges Care Coordination Collaborate with physicians and interdisciplinary teams Accurately transcribe and implement physician orders Patient Advocacy Educate patients and families on care plans and health management Uphold patient confidentiality and resident rights at all times Diagnostic Support Collect and prepare specimens (urine, blood, sputum) for testing Qualifications Licensure: Active, unrestricted Massachusetts LPN license Education: Graduate of an accredited LPN program or nursing school Certifications: Current BLS / CPR and AED certification required Experience: Prior experience in long-term care or skilled nursing preferred Skills: Strong leadership and supervisory abilities Ability to make sound clinical decisions independently Effective communication and interpersonal skills For more details contact at [email protected] or Call / Text at 516-862-1169.
